Description

The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.

Responsibilities

  • This is an office based position in Doral, FL. Initial training will be 3-6 weeks in the local office. Training is temporarily WAH until further notice.
  • This position also requires home visits to members in the area and surrounding counties (once Covid-19 restrictions are lifted).
  • The Care Coach 1 employs a variety of strategies, approaches and techniques to manage a member's health issues
  • Identifies and resolves barriers that hinder effective care
  • Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning
  • Understands own work area professional concepts/standards, regulations, strategies and operating standards.
  • Makes decisions regarding own work approach/priorities, and follows direction
  • Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation
  • Bilingual English/Spanish with ability to pass a language assessment
  • Must be able to work Mon-Fri between the hours of 8am-7pm but be flexible with your hours based on business needs and hours could include weekends/holidays during the Medicare Annual Election Period, Sept 16-Dec 8.
  • Previous experience in phone intensive environment with strong customer service skills both inbound & outbound (80-90% at times)
  • Previous administrative or related experience
  • Current valid Driver’s license required & ability to travel up to 75% at times in the surrounding counties
  • Team player, ability to work with tight schedules, multiple priorities as well as work independently
  • Strong proficiency in Microsoft Office Programs Word, Excel

Preferred Qualifications

  • Associates/Bachelors/Master's degree
  • Prior experience with Medicare & Medicaid recipients
  • Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems
  • Experience with health promotion, coaching and wellness
  • Knowledge of community health and social service agencies and additional community
